How to Replace a Lost OSHA Card

Losing your OSHA card can prevent you from getting on the job site. Here is what you need to know to replace it.

Who to Contact

You cannot contact OSHA directly to get a replacement card. OSHA does not keep records of individual students. You must contact the authorized trainer or training provider who originally issued your card.

Time Limits

You can only request a replacement card if your training took place within the last 5 years. If it has been longer than 5 years, you must retake the course.

Fees

Providers typically charge a replacement fee (usually around $20 to $40) to process and mail the new card. You are only allowed one replacement per training class.
